GP Supervisor Information

Individual GP - Medical Student Supervision

Each individual GP who intends on supervising a student at your practice must submit a separate form advising of their availability and selection preferences.

Shared GP - Medical Student Supervision

For those practices who supervise students with a number of GPs sharing supervision of a single student on placement please complete the additional GP Supervisor's details section providing each GPs details so that we are aware this will be a shared supervision between the Doctors at your practice.

If  there are more than three GPs that may be supervising one student over the course of the placement, please contact the Clinical Placements team directly to assist with the submission. It is preferrable the student is provided with consistent supervision throughout the course of the placement and has one consistent GP supervising and signing off on the students end of placement assessment, we do not encourage more than three GPs assigned to one student throughout a placement.
